DO voltage fluctuates around 0.5V, does not respond to commands

I have been testing a DO with the Core3+ and for a couple weeks everything has worked as expected. I'm using only 2 of the outputs (O_1, O_2) to trigger relays as needed. Today, I've noticed a problem. When any outputs are set to 0, a multimeter reading shows the voltage hovering around 0.5V - 0.6V. When I set the output to 1, the voltage still only fluctuates in this range--no change. I've tried two mutlimeters, and when there is no power going into the system all the outputs show 0V as expected. Before, when everything was working, they would read almost exactly 0V when low and 12V when high. I'm not sure what change could have caused this, but it affects all of the output pins. Is this a known issue? Is something misconfigured?
